Ketamine Treatments

Spravato:

Indications for use: Treatment resistant depression where the individual has tried and had inadequate response to two antidepressant trials. 

Spravato (es-ketamine): the only FDA approved nasal spray for adult with treatment resistant depression, reduces depression symptoms when two or more antidepressants haven't worked.  Spravato is an NMDA receptor antagonist believed to work differently from transitional antidepressants by acting on glutamate.  While the exact role of glutamate in depression is unknown, it is a chemical messenger found in the brain that is thought to help balance well-being and depression. Spravato can be taken with or without an oral antidepressant.  

Cost: Treatment is processed through insurance.  Prior to your appointment your provider will request your insurance information to determine eligibility and cost.  Upon the Assessment for Spravato this will be reviewed with you before treatment begins.  Although each insurance company is different, treatment is usually well covered and in many cases is $0.

What are some indications for use?

Depression, anxiety, and addiction just to name a few

The most commonly prescribed antidepressant medications are called selective serotonin reuptake inhibitors (SSRIs). Only about 1/3 of people with major depressive disorder achieve remission after treatment with SSRIs. If the first SSRI is not effective, a different one can be tried or a second one can be added. Almost 70% of people with depression will be able to achieve remission after trying different SSRI options or combinations. Unfortunately, the remaining 30% of people will not be able to find a treatment that works and are said to have treatment resistant depression (TRD).

According to research studies, ketamine treatments can be an effective treatment for 70 to 80 % of individuals with TRD. Unlike SSRIs, which can take weeks or months to begin taking effect, a single dose of ketamine may produce antidepressant effects within a few hours. Although the effects of ketamine usually last for several weeks, some people can remain depression free for months.

Cost: 

Weeks 1-4 = 1600 or 1800 depending on if you are a new patient or existing patient.  

Weeks 5-8 = 800 

Weeks 9-12 = 400

Total = 2800 or 3000 depending on if new or existing patient

 

Additional treatment recommendations:

Research has shown that therapy done within 48 hours of Spravato or Racemic Ketamine is given improves neuroplasticity and helps to integrate the changes to mood.  Although not required for treatment, it is recommended that therapy is done in tandem with medication treatment for best results.

Additional education:

Racemic Ketamine and Spravato is generally safe when administered under the supervision of a licensed, certified provider specially trained in appropriate management. Prior to receiving a dose, an initial assessment is performed. Your heart rate, blood pressure and oxygen level are monitored during treatments. Ketamine has a remarkable safety profile that does not slow the patient’s breathing or heart rate like other anesthetic medications. For this reason, it is safely utilized in surgery for adults, pediatrics, obstetrics, trauma and veterinary medicine. The doses used to treat depression and other mental health conditions are subanesthetic or much lower doses than required for anesthesia. During the treatment, patients may feel some mild dissociation or detachment from their environment. Patients may also experience a temporary increase in blood pressure and heart rate, which are monitored. Other possible side effects include: increased salivation, nystagmus or blurred vision and nausea. All side effects are gone approximately two hours after the treatment. 

What form of treatment will I receive?

Each individual is different and therefore a personalized review of history, medications, and goals is necessary to determine which formulation of medication will be recommended.  

The two formulations of ketamine are provided in either Racemic Ketamine (a mixture of the two forms of Ketamine being es-ketamine and ar-ketamine) or Spravato (es-ketamine only).  Spravato is the only FDA approved formulation of ketamine and Racemic Ketamine is considered "off label" treatment but is still regulated and monitored by accrediting bodies..

  • In your initial visit the provider will evaluate your full medical, psychiatric, psycho-social, and family history in conjunction with your present symptoms. An accurate medical history is important to obtain at this first visit because certain medical conditions can present with psychiatric symptoms. Prior to this visit you will be sent a link to register with our patient portal which includes New Patient Questionnaires for your online account. The patient portal also allows for easy and necessary communication between you and your provider. During this initial session, your treatment goals will be discussed in depth and a full treatment plan (including medications, complementary and alternative wellness interventions, and/or psychotherapy if needed) will be prescribed. We offer in-person and telehealth services. Telehealth and our patient portal are readily accessed from any smart phone, tablet, and/or computer.

  • In your initial evaluation our providers will work with you to establish your goals and create a treatment plan to support your goals. While some people benefit from ongoing psychiatric care other people may prefer to be seen less often.
